He was predeceased by: his son H. John Pawlak III (Teresa). He is survived by: his wife Darlene (Smith); his daughters, Yvonne Muse (Don) and Theresa Harris (Matt); and his stepdaughter Cheryl Hubbard (Keith Sr.). He is also survived by nine grandchildren and eleven great grandchildren.

In lieu of flowers, the family is asking for donations to the Shriners Children's Cancer Research Fund.
