She was predeceased by: her parents, Lloyd Osteen and Myrtie Osteen. She is survived by: her grandson Joshua LePage; and her siblings, Carolyn Rogers (Dale), Jacquelyn Gornto and Dr. L. D. Osteen (Brenda). She is also survived by 6 nieces and nephews and many friends.
In lieu of flowers the family requests donations be made to St. Jude Children's Hospital, Shriners Hospital for Children or Community Hospice of Palatka, who took such compassionate care in Phyllis's final days.
