He is survived by: his parents, George Baker and Mary Baker of Chittenango, NY; his children, Justin of Baldwinsville, NY and Mackenzie of Tully, NY; his fiancé Allison Cloutier; and his sisters, Lisa Rowland and Lory Wittwere of New York. He is also survived by several nieces and nephews.
After his unexpected passing, Jay continued helping others through the donation of tissue and organs.
