He was predeceased by: his parents, Kenneth J. O'Sullivan and Gertrude O'Sullivan (Chick); and his sister Janet Hagget. He is survived by: his children, Cheryl Averill, Donna (David Razin) and Kevin O'Sullivan; his grandson Richard Averill (Emily); his daughters, Khloe, Caitlin Averill and Jenna (Chase Thornton); his grandchildren, Landon, Allie Jane, Amanda Dargy and Shoshanah Razin; his sibling-in-law Courtney Haggett; and his wife Janice (Goode).
In lieu of flowers, memorial contributions may be made to Cornerstone Hospice and/or the Missions funds at Life Church.
