He is survived by: his son Liam Andrew Smith; his parents, Paul Smith (Brenda) and Pam Wilson (Fred); his siblings, Sam Smith, Jessica Reffner, Jaymie Aubert (Matt), Mari Treverton (Kenny), Craig Stokes (Lauren) and Shayne Stokes (Samantha); his uncle David (Annie Hunsucker); his aunts, Debra Thompson (Dave), Martha Gates (Roger) and Mary Logsdon (Tommy); and also Gabriella Nunes. He is also survived by several nieces and nephews.

In remembrance of JD Smith's life, the family asks that any charitable donations be made for the future well-being and education of his son, Liam.
