He was predeceased by: his parents, Johanna and Gerrit; and his grandson Joshua. He is survived by: his wife Hazel DeBorst; his daughters, Jennifer Mook (Gary) and Susan O'Neill (Michael); his grandchildren, Garrett (Rebecca), Paul (Amy), Matthew, Jill, Kaitlin, Austen, Tyler, Devon and Johanna; and his brothers, Cees (late Paula), Adrie (Inge) and Johan (Mia). He is also survived by nieces and nephews.
