He was predeceased by: his wife Susan. He is survived by: his granddaughter Amber; his sons, Andrzej of Warsaw, Poland and Kristoff Kolakowski of Warsaw, Poland; his sister Ginger Scrofani of Hendersonville; and also Sam.
