He is survived by: his wife Susan; his daughters, Leigh Weinraub (Miles Horton) and Stefanie Weiss (Jonathan); his grandchildren, Benjamin Weiss, Allyson Weiss, Jordan Weiss, Nicholas Horton and Noe Horton; and his sister Gari Weinraub.
The family suggests donations in Perry's memory be made to Dr. John Bookvar, to support brain tumor research at Northwell Health Foundation.
