He was predeceased by: his daughter Heather Ellsworth. He is survived by: his parents, George Ellsworth and Jan Ellsworth; his wife Anita Ellsworth (Nee Riner); his son Dakota Ellsworth; his stepson Tyler Fields; his siblings, David Lingerfelt and Wendy; and his niece Jasmine Villa.
