He was loved and cherished by many people including: his parents, Fred Polk and Lola Polk (Raulerson); his wife Gwen; his children, Fred, Ray Polk (Margaret) of Dade City, FL, Tim Polk (Sharon) of Zephyrhills and Sandy Polk Hayes (Barry) of Lakeland, FL.; his grandchildren, Kelly Smith, Shannon Jeter, Justin Polk, Tommy Polk, Alicia Guanio, Michael Polk, Bobby Polk, Christian Polk, Timmy Polk Jr., Chase Freeman Oliver Simonds and Chelsea Ryan; and his great grandchildren, Joshua, Makenna, Ty, Todd, Nathan, Cauy, Carly, Matt, Julie, Connor, Aiden, Savanna, Kaya, Houston, Hardy, Mason, Ella, Elijah, William, Brynleigh, Patrick, Savannah and Izzie. He was also cherished by 4 children.
