He was predeceased by: his parents, Tommy Collinsworth and Gurthey Lee Reynolds Collinsworth; his wife Sara Elizabeth Grice Collinsworth; his son Jed Collinsworth; and his granddaughter Rachel Cooper.
He is survived by: his children, Paula Collinsworth Joyner (Jim), Paulene Cooper (Bryan) and Jan Crosby Green; his granddaughters, Victoria Cooper and Priscilla Burdeshaw; and his great grandchildren, Travis Levi Cooper, Noah Paul Cooper, Levi LeRae Burdeshaw and Ripley LeRae Downing. He is also survived by Numerous nieces, nephews and cousins.
