She is survived by: her son Lamar Formby; her grandchildren, Mark Johnson, Laura Johnson, Joshua Formby, Job Formby, Grace Elizabeth, Julia Zachary and Jocelyn Marie; and her great grandchildren, Adeline Brown Johnson, Micah Asher Formby, Jonah Mavyn Formby, Ella Ezme Formby and Jace Havox Formby.
