He is survived by: his sons, Anthony Church of Bonifay, FL, Dominic Church of Cottondale, FL, Vincent Church of Bonifay, FL, Chad Cullen and Billy Nicholas of Cottondale, FL; his brother Eugene Church (John Church) of Hartford, AL; his nephew Matthew Church of Graceville, FL; and his nieces, Helena Church of Samson, AL and Charlie Ann Church of Hudson, FL. He is also survived by 9 grandchildren.
