He was predeceased by: his father Henry L. Smith. He is survived by: his mother Cathy L. Thomas; his significant other Jessica Cloud; his children, Hailey Smith, Carlie Smith, Jacob Smith, Jocelyn Smith, Alexa Smith and Samantha Smith; and his brothers, Scott L. Smith (Marilyn), Philip H. Smith (Irene) and Clifford Smith.
