He was predeceased by: his parents, Pete Slavik and Anna Slavik; his wife LaVerne Norma Slavik; and his daughter Cathy Jo Wheeler. He is survived by: his children, Paul Slavik (Laura) of Middletown, Joseph Slavik of Christiana, Theresa Klein (Scott) of Roslyn, NY and Lisa Chamness (Tom) of Baltimore, MD.; his son-in-law Kenneth Wheeler of Rome, GA.; his grandchildren, Dwayne Wheeler, Jennifer (Craig Wilhelm), Reilley Chamness, Hannah, Sarah Klein and Will Slavik; and his great grandchild Gregory Wheeler.
