He was predeceased by: his sister Dawn White. He is survived by: his wife Elaine Ferguson (Pilat); his daughter Lauren Kozlowski (Peter); his grandsons, Peter "Tre" J. Kozlowski, III and Andrew Wade Kozlowski; his niece Jennifer Marie Whaley (Joseph Andrew); his great-nephews, Justin Andrew and Jason Patrick Whaley; and also, Christopher Patrick White and Niles Theodore Pilat.
