He was predeceased by: his parent Herman Cook; his wife Marie Cook; and his granddaughter Crystal Lynn Johnson. He is survived by: his parent Florence Cook (Carey); his children, Guy Cook (Betty) of Clayton and Dale Dittman of Clayton; his grandchildren, Amy Dittman, Betty M. Cook, John R. Cook and Harry Dittman, Jr.; and also John H. Cook.
