He was loved and cherished by many people including: his parents, Salvatore Formica and Rachel Formica (Lucibello); his wife Michelle Formica (Rinzivillo); his daughter Lisa Venice; his grandsons, Thomas and Zachery; his nieces and nephews, Leonard Ciavarella, Jr. (Lisa), Sheila Murray (Dan), Lynn Ciavarella, Donald and Eric Iannucci; and his sisters, Amelia Ciavarella and Katharine Iannucci.
