He was predeceased by: his parents, Anthony Sr. Kozlosky and Rosemarie Kozlosky (Dery); his brother Allen Kozlosky; and his daughter Ashley Kozlosky. He is survived by: his wife Allison Kozlosky (Ives); his daughter Tammy Braeuer; his stepdaughter Camryn Conroy; his sister Cheryl Wilson; and his pet Harley.
