He was predeceased by: his mother June Lee Nelson (Newell) of Jamaica. He is survived by: his father Clive Mondale Perkins of Jamaica; his sons, Jason M. Perkins, II of Vineyard Haven, MA and Kahlil E. Perkins of Windsor; and his siblings, Kevin O'Neill Smith, Pedro Perkins and Cassandra Perkins.
