He was predeceased by: his parents, Frank Formanek and Emily Formanek (Orlando); and his brother Stephen Formanek. He is survived by: his wife Susan Formanek (Cusick); his children, Frank Formanek (Lisa) of Colchester and Sarah Chase (David) of Bradenton, FL; his grandchildren, Taryn Formanek, Carsyn Formanek, McKayla Chase, Logan Chase, Kailyn Formanek, Evan Coakley and Ryan Coakley; and his brother Paul Formanek (Linda) of Suffield. He is also survived by many nieces and nephews.
