He was predeceased by: his parents, Clark Cook, Jr. and Mary Cook (Crichton); and his brother Ronald Cook. He is survived by: his daughter Kathleen Cook; his siblings, Kevin Cook, Jacqueline Traver and Cindy Cole; and his grandchildren, Craig, Gabriella and Aaron "AJ". He is also survived by several nieces and nephews.
