He was predeceased by: his spouse Beverly Waldo; and his brother David Waldo. He is survived by: his daughter Tammy B. Coutu of Tolland; his cousin Donna Enman of Tolland; and his nieces and nephews, Carol Kozlowski of Vernon, Terry Danise (Howard) of Ellington, Michael Kozlowski (Heather) of Vernon and Jeffrey Kozlowski (Ellen) of Tolland.
