He was predeceased by: his parents, Walter Kozak and Helen Kozak (Parzych); and his son Robert Kozak. He is survived by: his wife Cynthia A. Kozak (Carlisto); his children, Ami Hughes (Kevin), Jeff (Ellie), Jason Lafleur (Stacy), Adam, David, Dionna, Alycia, Gordon and Zeni Gushue (Matt); his grandchildren, Chase, Aidan, Veronica, Chloe, Mason, Amber, MacKenzie, Madison, Kolton, Alaris, Skylar, Easton, Kitai, Jayden and Kiara; his brother Ronald (Diane Kozak) of Holyoke; his nephew Christopher Kozak; and his niece Kimberly Kenwood.
