He was predeceased by: his parents, Bertha McIntosh and Carl McIntosh; and his nephews, Christopher Pender and Michael Pender. He is survived by: his wife Theresa Mcintosh; his children, Sandra Archuleta (Jose), Andrew Segura, Michelle Bly (Dale) and Richard Mcintosh; his grandchildren, Jaden Archuleta, Lexie Archuleta, Nathaniel Bly and Ally Bly; and his siblings, Tracy Pender (Marsha), Kerry Pender, Danny Pender (late Derri) and Belinda Dominguez (Frederick). He is also survived by several nieces, nephews, great-nieces, great-nephews, aunts, uncles, and cousins.
In lieu of flowers the family requests donations be made to New Life Children and Youth Ministries to benefit the future generations to come.
