She is survived by: her significant other Donald Hoffman; her children, Jerry Forrest, Paul Hoffman (Linda) and David Hoffman (Roxanne); her grandchildren, Heidi Henaire (Kristopher), Allison Forrest-Laiuppa (David), Ian Hoffman, Robert Chasse (Robin), Michael Chasse (Terri) and Jason Chasse (Rayanne); her great grandchildren, Kara, Autumn, Ethan, Jillian, Joey, Johnny, Colton, Ella, Kayden and Willa; and her siblings, Joyce Hames (Les), Linda Helming (Terry) and Alan Knopf (Mary).
