He was predeceased by: his wife Helen T. Slovak (Zdravecky); his parents, John Joseph and Francis Slovak (Janack); and his sisters, Mary Turcek and Anna Matula.

He is survived by: his children, John "Jack" Slovak, Joseph J. Slovak, Jr. (Deborah) and Janet A. Hiltz (Gorden Jr.); and his grandchildren, Sara Slovak, Ryan Slovak, Brianne Lopez (Hiltz), Gorden Hiltz, III and Geoffrey Hiltz. He is also survived by many nieces and nephews.
