She was predeceased by: her husband Jesus; her parents, Stefan Szmajda and Anna Syrotiak Szmajda; her husband Wasyl Zazulak; and her siblings, Maria Szmajda Koch and Andrej Szmajda.
She is survived by: her daughter Ann "Hania" Zazulak of Shelton; and her sister Janka Kolebuk of Poland. She is also survived by several nieces and nephews.
