She was predeceased by: her father John Kopak; and her brother Johnny Kopak. She is survived by: her husband Bob Evans; her daughter Kris Phalstaf; her son-in-law Bradley Switt of Wales, MA; her son Rob; her daughter-in-law Michiko Evans of Japan; and her granddaughters, Kana, Nami and Cece.

Donations in her name should be made to the Gardner Lake Volunteer Fire Department, 429 Old Colchester Rd., Salem, CT 06420 in lieu of sending flowers.
