He is survived by: his wife Esther Kozak (Derlaga); his daughter Kyra Woods; his grandchildren, Chelsea and Jacob Woods; his niece Rebecca Yankowitz; his siblings, Joseph Kozak, Barbara May, Patricia Brady and Mary Jane Kozak; and his sister-in-law Carolyn Yankowitz.
