He was predeceased by: his wife Mary Ann Szpak Patulak; his parents, Alexander Patulak and Stella Krystofik Patulak; and his sister Elizabeth Patulak. He is survived by: his sons, Robert Patulak of Prospect, Jeffrey Patulak (Mindy) and Thomas Patulak (Kimberly); his grandchildren, Tyler, Sydney and Maleah Patulak; and his sister-in-law Andrea Dixon. He is also survived by several cousins and friends.
