She was predeceased by: her parents, Frank Kozuch and Sophie Kozuch (Gotzna); her daughter Susan Schwab; her siblings, Helen, Eddie, Benny, Frankie Kozuch, Julie Hinchcliff, Elizabeth Talbot, Evelyn Bogush and Mary Bushley; and her siblings-in-law, Mary D'Addese and Harry D'Addese.
She is survived by: her husband Anthony E. D'Addese; her children, Jackie D'Addese of Bristol, Kathy Joslyn of Farmington and Joseph D'Addese (Yvonne) of Bristol; her grandchildren, Steve Joslyn (Robyne), Krysten Robbins (Jimmy) and Sarah D'Addese; her great grandchildren, Grady, Kinley Joslyn, Ryleigh and Landon Robbins; her sister-in-law Josephine D'Addese; her son-in-law Stewart Schwab; and her friend Jackie Dionne. She is also survived by several nieces and nephews.
