She was predeceased by: her father Donald Pavlak; and her sister Suzanne Pavlak. She is survived by: her mother Mary Ann Pavlak of Norwich; her daughter Jennifer Simmons; her son-in-law Edward Simmons of Dothan; her brothers, Donald Pavlak of Brainard, Timothy Pavlak of Greeley and Thomas Pavlak of Le Roy; and her granddaughters, Victoria Simmons of Dothan and Samantha Simmons of Dothan. She is also survived by many Aunts, Uncles and Cousins.
