He was predeceased by: his grandparents, Ruth, Donald Smith, Liuntina and Custodio Cruz. He is survived by: his parents, Sandra Smith and Robert Smith; his siblings, Christopher Smith and Hollee Chontos; his fiancé Carine Bonnist; his children, Hunter, Hayden, John Robert, Bella, Sage and Dakota; his aunts and uncles, David, Karen Smith, Philip and Sandy Cruz; and his nephews, Chase, Cole, Trevor Smith and Colin Chontos. He is also survived by several cousins, extended family, and countless friends.
In lieu of flowers, donations in his memory may be made to Ability Beyond, Alzheimer's Association of Connecticut, and the Andrew Cruz Endowed Chair in Palliative Care.
