He is survived by: his wife Amy; his son Cameron; his siblings-in-law, Wade Gradia, Tracy Gradia and Darla van Nieukerk; his parents, James Kozlowski and Erin Kozlowski; and his siblings, Brad (Amanda), Lisa and Lori Kozlowski. He is also survived by his family, friends.
