He was predeceased by: his parents, Anthony P. Fazzone and Laura DeLuca Fazzone; his stepfather Gaetano "Guy" Clomiro; and his brother Andrew C. Fazzone, MD. He is survived by: his wife Kathy; his sons, Kris (Meg), TJ (Tabitha) and Jon (Huilin); and his stepbrothers, Joseph and Vincent Clomiro. He is also survived by ten grandchildren, and one honorary granddaughter.
