He was predeceased by: his parents, Alvin "Al" Kollmann and Maxine "Mickie" Heather Kollmann. He is survived by: his wife Karen; his sons, Kyle, Bradley (Daniella), Justin, Evan and Trevor; and his granddaughter Kennedy Juliet Kollmann.

In lieu of flowers, memorial donations in David's memory may be made to St. Peter's Grace Episcopal Church.
