He was predeceased by: his brother Raymond Hawkes; and his parents, Walter Hawkes and Doris Hawkes (Bruce). He is survived by: his wife Shirley; his daughters, Karen (Daniel McAvoy), Kathleen (Charles Imbergamo), Kimberly (Matthew Weidman) and Kelly (David Wasserman); his grandchildren, Kayla (Jack Lynch), Sean, Meghan McAvoy, Alicia, Nathan Imbergamo, Erin, Benjamin, Eric Weidman, Ryan, Daniel and Jenna Wasserman; and his siblings, James Hawkes and Beverly Blondin.

Due to Covid protocols, all in attendance are required to wear a mask.

In lieu of flowers, donations can be made to Friends of Hammonasset (https://hammonasset.org) or the American Cancer Society (https://www.cancer.org), in honor of Jerry.
