He was predeceased by: his parents, Eudokia Polak and Wasyl Polak. He is survived by: his wife Helena Melnyk Shulewsky; his son Paul; his sister Maria Cezus of Concord, GA; and his stepfather Wasyl Shulewsky. He is also survived by brothers and sisters-in-laws, nieces and nephews, and cousins.

Donations may be made in Michael's memory to St. Josaphat Ukrainian Catholic Church.
