He is survived by: his wife Donna Ann Puchalsky (Smith); his parents, Lawrence Puchalsky and Barbara Puchalsky (Hornick); his sons, Lawrence Puchalsky III and Nicholas Winfred Puchalsky; his siblings-in-law, David Smith, Linda Gorman, Frances Ouellette and Sandra Smith; his niece Rachael Kennedy (Kevin); his uncle Larry; and his siblings, Lynn Olszewski, Janice Carson, Gail Puchalsky and Kenneth Puchalsky. He is also survived by many loving nieces, nephews and friends.
