He was predeceased by: his father Dr. William Stevens. He is survived by: his mother Sallie Stevens; his wife Dr. Leila Garrett-Stevens; his sons, William and Owen; his sisters, Kristin Stevens, Stephanie Stevens and Heather Clayton (Carter); his nieces, Erin Tippett (Brian), Emily, Ella Clayton, Sarah (Adrian), Charlotte Robinson and Mallory O'Shea; and his nephew Tristan O'Shea.

In lieu of flowers, the family requests donations be made to Camp Dudley, Westport, NY, https://www.campdudley.org in Mike's name.
