He was predeceased by: his child Thomas Kozlak; and his parents, Charles Kozlak and Norma Kozlak (Bunn). He is survived by: his wife Margaret Kozlak (Backiel); his children, Jonathan Kozlak (Alison Kozlak) of Burlington, CT, Daniel Kozlak of Niantic, CT and Lisa Rettenmeier (Michael Rettenmeier) of Burlington, CT.; his grandchildren, Lauren, Kerry, Julia Kozlak and Benjamin Rettenmeier; his sister Linda Hauch (late Eric Hauch); and his sister-in-law Jane Kozlak (Zacchera). He is also survived by several brothers and sisters-in-law, nieces, nephews and cousins.
