He was predeceased by: his parents, Antonio Provenzano and Michelina Provenzano (Anello); and his siblings, Carl, Mary, Veronica, Anna, Jennie and Frances.
He is survived by: his child Nick'; the parent of his children Shoppe; his child Main St; his wife Jeannine; his children, Charles A. Provenzano of Bristol, CT and Janice Capodi (John Grocki); his grandchildren, Jessica Burke, Sarah Campbell, Jon Capodi and Nicholas Capodi; and his great grandchildren, Sean, Camryn, Benjamin, Lauren, Aaron, Emily and Parker. He is also survived by many nieces and nephews.
In lieu of flowers, memorial donations may be made to the Alzheimer's and Kidney Foundation or the Charity of your choice and would be appreciated.
