He is survived by: his grandmother Baker; his wife Nancy Hayes (Gates); his son John (Lisa); his grandchildren, Samantha, Kelsey (Mark) and Abigail (Matt); his children, MaryBeth and Mark (Beth); his grandchildren, Mark and Emma; his children, Kat, Jack, Annabelle, Tessa, Layla and Owen; his nephew Phil (Holly); his pet Velvet; his parents, Jack and Freida; his brother Buz; and also The Reverend Mary Beth.
In lieu of flowers, his family requests that donations be made to Masonicare Home Health and Hospice, 33 N.Plains.
