He was predeceased by: his parents, Art Chouinard and Doris Chouinard (Gaudet). He is survived by: his former wife Sally Chouinard of Canton, CT; his children, John Chouinard of Bristol, CT, Mary Ellen Chouinard Beauchamp (Robert Beauchamp) of Berlin, MA, Paul Chouinard (Clara) of St, Nancy Norander (Phil) of Las Vegas, NV and Tom Chouinard (Kathy) of Canton, CT.; his grandchildren, Jarrett, Emily, Danielle, Amanda, Cole, Kayla, Kyle and Olivia; and his brother Don Chouinard of Vernon, CT.. He is also survived by several nieces and nephews.
In lieu of flowers, donations could be made to the Alzheimer's Association.
