He was predeceased by: his father Garold Middlemist. He is survived by: his mother Geraldine Middlemist; his wife Kim Bales; his daughters, Emma and Ellen; his siblings, Greg Middlemist (Debbie), Kathy Currier, Elizabeth Harless (Steve) and Theresa Lewis; his parents-in-law, Gary and Carol Bales; and his brothers-in-law, Gary Bales (Michelle) and John Bales (Cathy). He is also survived by many nieces and nephews.
