He is survived by: his parents, Thomas Forehand and Alicia Forehand; his wife Tracy Alexander; his son Aaron (Mandi); his granddaughters, Ariel and Evangeline; his brothers, Danny (Patti) and Gary; and his nieces and nephews, Luke (Mindy), Britany, Casey (Savannah), Derek and Shannon. He is also survived by in-laws; and many friends.
