He was predeceased by: his parents, John Duggan Mayhoffer and Kathleen Ada Weckmueller; his stepmother Jeane; and his brother Robert. He is survived by: his wife Tami; his sons, Edward (Lacie), Michael (Heather) and Philip (Sarabeth); his grandchildren, Briley, Karsyn Mayhoffer, Jacelyn, Sawyer, Tayson, Whitley, Rynlee, Kyden Mayoffer and Edison Mayhoffer; and his sister Leannah Baron (Rick).
