He is survived by: his wife Kathi; his former wife Karen; his children, Dr. Alexis Kotelko, Nick Kotelko (Angie), Dr. Cody Kotelko and Kaitlyn Kotelko; his brothers, Zenon Kotelko (Michelle) and Terry Kotelko; and his pets, Milo and Gracie.

In lieu of flowers, Memorial contributions can be made to a charity and ministry that was near and dear to Dennis' heart.
