He is survived by: his wife Connie Provick; his children, Jerome Eickstaedt, Jordanna Wild, Samuel Provick, Jamie Graif and Jesse Provick; his parent Donna Rosen; his siblings, Jaynee Provick and Cameron Provick; his grandchildren, Blake Austin, Johnny Graif, Michael Graif, Molly Wild, Sadie Wild and Mary Lue Graif; and also Allan Rosen.
All memorial donations will be given to charities supporting suicide prevention.
